What is the best way to eliminate the deep face wrinkles?

Suppose that a young man has very deep forehead wrinkles that have improved with none of the forehead rejuvenation non-surgical methods. He went online and searched about endoscopic forehead lift, but all experienced surgeons suggested that he avoid an endoscopic forehead lift. Why?

He only wants to eliminate the forehead’s deep wrinkles, and an endoscopic forehead lift can only stretch the deep layers of the forehead. This stretch is not to the extent that can be transferred to the subcutaneous layer and smooth such a thick skin. Ugliness and aging signs in forehead

Eyebrow drooping

Droopy eyebrows are one of the most common signs of aging in the forehead. Men experience droopy eyebrows more and sooner than the women. The signs can be reduced in the initial stages of this drooping via Botox injection and using other non-surgical methods, but finally, most persons become the candidates for brow lift surgery. This surgery can be an endoscopic or open surgery. In order to select the best method, one had better pay attention to other signs that appear with aging.

The men who have hereditary low eyebrows may become candidates for forehead and brow lift surgery at a young age (under the age of 30). Due to thick skin and inflexible soft tissue of eyebrows, non-surgical methods are not usually very effective in such situations. However, we had better try one or more non-surgical methods to correct it.

Forehead wrinkles and frown line

Among the most common signs of aging are forehead wrinkles and frown line. Fortunately, most of these wrinkles (especially, frown line) respond well to Botox injections in the early stages and the person needs to take no other action for a long time. Filler injection beside Botox injection can yield better results. Non-surgical methods are ineffective in some people. In those with very deep wrinkles, forehead lift surgery can be a great help.

Drooping of the eyebrow tail and temporal region tissues

There is no lifting muscle in the lateral parts of forehead to lift the temple tissue and eyebrow tail, and the upper eyelid circular muscle and the gravity cause them to droop instead. Some people have a low eyebrow tail genetically. These people also develop signs of the drooping tissues in the temple area during adolescence. The drooping signs of eyebrow tail and temporal region tissues can be corrected in the early youth years through temple lift surgery. Temple lift candidates are often adolescents who undergo this surgery to elevate the eyebrow tail and change the area around the eyes.

Forehead lengthening

 An increase in the length of the forehead is an obvious characteristic of aging, especially in men. The distance between the eyebrows and the hair (forehead length) increases with the hair loss and receding of the hairline. Besides ugliness, this can alone cause people to look 5-10 years older than their real age in those with severe hair loss and it adversely affects their mood.

Fortunately, hair transplantation can solve the problem of most men. Some women do not want to do hair transplant and prefer to have a shorter forehead without hair transplantation (as the implanted hair is often thicker than the frontal forehead hair and may reveal the hair transplantation secret in women). Forehead shortening surgery can be considered in such cases and other forehead corrections (e.g., temple lift, brow lift, forehead protrusion correction) are simultaneously performed.

Forehead protrusion

The protrusion of the lower forehead results from the excessive growth of forehead sinuses.  These sinuses’ growth usually continues during puberty until the bone growth plate is closed. However, its growth continues sometimes after this period. Fortunately, this can be seen less in women and they do not usually need to undergo surgery.  A slight protrusion of the forehead is acceptable in men, but if this protrusion is large, especially in people with droopy eyebrows at the same time, the person’s face appears a little violent and angry and this creates an unpleasant feeling in the person.

Temple’s tissues atrophy

The muscle and fat atrophy in the temporal region is commonly seen with aging. Fortunately, most of these atrophies are mild and can be corrected by fat injection or filler injection. Temple prosthesis is used in extreme temple atrophy cases in which one does not have even adequate fat for injection.

Other changes caused by aging

Besides the above mentioned changes that many physicians and people in the community are aware of, other changes may occur in the forehead as we get older. The reason why these changes receive less attention can be attributed to the following factors:

  1. Not all people develop these signs and symptoms.
  2. These changes are seen as a paradox (i.e., the opposite of the normal state) in some cases.

The most important unknown or less known changes occurred in the forehead are as follows:

In some people, not only the eyebrows do not descend by aging, but also they move upward as compensation. Therefore, face lift is ineffective for them and proportionality between forehead elements should be established.

A relative protrusion is observed in some people due to relative atrophy of the surrounding tissues and the small growth of the main area. This state is more known in some facial parts. In some elderly people, we see that the auricle and nose continue to grow despite the facial tissues atrophy. The edge of the orbital (eye) socket is among parts that will become relatively protruded. This protrusion is still evident despite fat injection, forehead lift, and eyelid surgery. The approaching of the eyebrows and forehead tissues to the midline in some people is the most important change occurred in the brows with age.

What are the cosmetic surgical methods of the forehead?

Now we have investigated the ugliness indicators, let’s evaluate the methods used to correct them:

  1. Forehead lift surgery through open or coronal forehead lift

 It is a classic method for forehead lift that can correct all forehead and eyebrow defects (except long forehead) simultaneously. This method makes the forehead a little long and is especially more suitable for people with a low hairline. For some time after surgery, one may have an itching or decreased sensation in the head area. This reduces over time. This lift is more durable than the brows endoscopic lift, but people who like to have small changes in a narrow area of forehead can use other methods.

  1. Forehead lift surgery through open surgery by placing an incision along the anterior hairline (pretrichial forehead lift)

This method does not make the forehead long, but shortens the forehead at the same time. As one of the possible complications, the surgery scar trace may remain this way. Using some methods, we can prevent the formation of scars and fade them away.

Note: The surgeon’s experience to prevent the appearance of scarring post-surgery is of paramount importance. The difference between surgeons in performing this method is due to the difference between their experiences.

  1.  Forehead and brow lift through mid forehead brow lift

Mid forehead brow lift is the best method to correct the very deep wrinkles of the forehead. We can change the eyebrows shape to some extent using this method.  It can be easily done under local anesthesia.  Due to its low usage, the mid forehead brow lift has been forgotten. However, it eliminates the ugliness to some people in the best way.

  1. Direct brow lift

Direct lift of the eyebrows is the most durable method in the brow lift. It can change the eyebrow’s shape better than any other method if it is done correctly. Despite the risk of a surgical scar, the scar can be hidden in the best way for most people. New changes made to this method have increased its efficiency and it is recently performed more commonly.   1. Endoscopic brow lift

This method is suitable for people who want to undergo brow or temple lift with the smallest surgical incisions and do not have other forehead defects, such as forehead wrinkles, bone protrusion, and excessive length of the forehead, or are not willing to correct them. This method has been very welcome today. One of the reasons is the short duration of this surgery as compared with the open surgery. The incision of this surgery can be easily hidden within hair.  In people who like to use endotine, this device can be used instead of sutures to fix the tissue.  Although the result of endoscopic brow lift is more durable than open surgery, its repeatability allows us to repeat it again in the future whenever necessary.
